Related Service Logs Notice

 

Children who are eligible to receive special education and related service may have specific related services included in their Individualized Education Plan (IEP). Related services are added to an IEP if the IEP team determines that the specific related service is necessary in order for the child to benefit from his or her special education program. Illinois Public Act 101-0515 requires local education agencies to make related service logs that record the type and minutes of related services administered under a child’s IEP. Copies of related service logs will be available upon request from parents or guardians at the annual review of their child’s IEP. Students with disabilities who do not qualify for an individualized education program, as required by the federal Individuals with Disabilities Education Act and implementing provisions of the School Code, may qualify for services under Section 504 of the federal Rehabilitation Act of 1973 if the child (i) has a physical or mental impairment that substantially limits one or more major life activities, (ii) has a record of a physical or mental impairment, or (iii) is regarded as having a physical or mental impairment
